It is usually diagnosed between the 6th and 7th decade of life, and it is presented as a testicular pain of varying intensity, swelling and occasional fever. The aim of this paper was to report a case of Xantogranulomatous idiopathic orchitis in a teenage patient and ultrasound applications. At the scrotal ultrasound examination a hipoecoic image in the lower pole of testis hypoperfused at Doppler examination. The biopsy was performed and the hystopatologic exam attested xanthogranulomatous orchitis considering that the hystopathologic exam showed a diffuse infiltratéd of xantomized cells with focal microcalcifications and some seminiferous tubules interposed.
