    Multidimensionalidade de significados da automutilação na adolescência: perspectiva de adolescentes, responsáveis e profissionais de saúde

    To analyze the meanings of the practice of self-harm in adolescence from the perspective of adolescents, their legal guardians and health professionals. Methods: descriptive, exploratory, qualitative study. Participants were 7 adolescents, 20 legal guardians and 6 professionals from a Psychosocial Care Center for Children and Adolescents in the central region of Brazil. Data were collected through individual interviews with guardians and professionals, and focus groups with adolescents. Thematic content analysis was used in data analysis. Results: the thematic category “Meanings of self-harm” emerged, covering seven subcategories: “Psychopathology”; “Social communication”; “Peer identification”; “Regulating emotions”; “Generated feelings”; “Intentionality of action” and “Dependency”. Conclusion: there is a disconnection of the meanings attributed to self-harm by adolescents, legal guardians and professionals. The different meanings in the understanding of self-harm can interfere with the assertiveness of the care offered by professionals to adolescents, in the perception of legal guardians regarding the need to seek help, and in adolescents’ motivation to adherence to care.

    Avaliação de programas de controle de infecções relacionadas à assistência à saúde: perspectiva crítica da RDC N°48/2000 da ANVISA

    Objetivo: avaliar criticamente Programas de Controle de Infecções Relacionadas à Assistência à Saúde (PCIRAS) em hospitais de médio-extra portes, quanto ao cumprimento dos critérios sanitários nacionais. Métodos: estudo transversal realizado em 18 hospitais com Comissões de Controle de Infecções Relacionadas à Assistência à Saúde (CCIRAS) dos estados de Goiás e São Paulo, Brasil. Para coleta de dados aplicou-se formulário online fundamentado nos itens de avaliação preconizados pela Resolução de Diretoria Colegiada (RDC) Nº 48/2000 - ANVISA. Para análise estatística utilizou-se exame da frequência e distribuição das variáveis (média e desvio padrão - DP). Resultados: as CCIRAS atenderam 100% dos itens imprescindíveis, 93,0% (DP = 5,8) dos necessários e 64,8% (DP = 32,5) dos recomendados. Os Serviços de Controle de IRAS atenderam 90,2% (DP = 16,1) dos itens necessários, e 77,8% (DP = 19,2) dos recomendáveis. Conclusão: itens imprescindíveis foram cumpridos, porém os necessários e recomendados apresentam diferentes graus de inconformidades, podendo comprometer a prevenção e controle de IRAS. A aplicação de roteiro baseado na RDC N° 48/2000 contribui para conhecer a realidade dos PCIRAS dos hospitais, contudo, essa normativa não estabelece percentual mínimo de conformidade, dificultando a interpretação dos resultados. Há necessidade de atualizá-la para instrumentalizar os órgãos fiscalizadores. however, this normative does not establish a minimum percentage of compliance, making it difficult to interpret the results. It is necessary to update it in order to provide tools to surveillance agencies.

    Amamentação e contato pele-a-pele no alívio da dor em recém-nascidos na vacina contra hepatite B

    Tem-se como objetivo comparar a combinação entre o contato pele a pele com a amamentação, ao contato pele-a-pele durante a vacina contra a Hepatite B em recém-nascidos. Ensaio clínico randomizado cuja amostra constituiu-se de 55 RN a termo divididos entre os grupos contato pele-a-pele GP, n=38), amamentação combinada ao contato pele-a-pele (GPA, n=27). A dor do RN foi avaliada por meio da mímica facial. Mensurou-se também a frequência cardíaca de todos os RN e frequência de sucção dos RN no GPA. Houve diferença estatisticamente significativa nos períodos de compressão e recuperação entre os grupos para os escores do NFCS. Os valores médios das FC foram significativamente mais elevados no GP na antissepsia/injeção. As sucções no GPA variaram de 10 a 28 sucções por minuto. A amamentação combinada ao contato materno pele-a-pele pode potencializar o efeito analgésico do contato materno pele-a-pele, contribuindo para uma melhor recuperação do RN após o procedimento.

    Perfil clínico e epidemiológico da infecção pelo HIV/aids em idosos

    The increasing HIV/AIDS number of cases for elderly people appears as a challenge, highlighting the need to observe about the quality of life of this population. Descriptive study to investigate the clinical-epidemiological profile of HIV/AIDS infection diagnosed in elderly aged 50 years old and over who were in a clinical follow-up at a referral hospital from January 2001 to December 2008. Data collection occurred through analysis of patient records between January 2008 and July 2009. From 208 medical records analyzed, 45,7% were women and 54,3% were man, age average 60,6 years old. The sexual exposure category was predominant and concerning CD4 counting, 68,2% presented level lower than 200 cell/mm3. The mortality rate was high (44,7%). This increasing number of HIV\AIDS in the elderly population is concerning and these results emphasize the need for better care to this group.

    Epidemiologia e qualidade de vida em indivíduos com neuropatia diabética dolorosa: uma revisão bibliográfica

    Painful diabetic neuropathy (PDN) is conceptualized as a pain started or caused by a primary lesion or a dysfunction or a transitory perturbation in the peripheral or central nervous system. This study aimed to analyze bibliographic production on epidemiology and impact of painful diabetic neuropathy on people’s quality of life. It is a bibliographical research of studies indexed in five sources of data, the descriptors used were: diabetes, diabetes mellitus, painful neuropathy, neuropathic pain, quality of life, SF-36, epidemiology and similarities in Portuguese and Spanish, between 1998 and 2010. Twenty-eight publications were selected. PDN prevalence ranged from 26,4 to 65,3%; and the pain was described as "burning", "tingling", " needleful" and "continuous"; intensity ranged from mild to severe, capable of generating depression in 35% of cases and affects mainly the energy, ability to walk and sleep. The PDN is frequent among people with diabetes mellitus and generates negative effects on quality of life for this population.

    Bases para o cuidado de idosos portadores de paralisia supra-nuclear progressiva

    Chronic-degenerative diseases affect significant proportion of elderly, constituting important public health problem considering the population aging all over the world. In particular, those who bring limitations and functional inabilities are the most compromise the quality of life, in particular, when they cause dependence for activities of daily life. With advancement of science, some these diseases are recent discoveries, bringing to professionals doubts as to their management. Progressive supra-nuclear palsy (PSP) is one of them and is the object of study of this actualization article, whose objective was to describe the PSP, as well as some aspects that give basis for care of elderly individuals with this pathology. The text brings initially information on the history of the disease, its prevalence, the etiology, diagnosis and clinical characteristics. Then discusses the need for an multidisciplinary approach in the monitoring of PSP bearer, as well as explores some questions related to cares that should be driven the family. Considering the rare nature of the disease, is evident need for health professionals are aware of the signs of installation of the disease to early diagnosis and monitoring initiated as quickly as possible, avoiding irreversible damage, particularly in relation to falls and aspiration.     Aspectos históricos da implantação de um serviço de atendimento pré-hospitalar

    The prehospital care service plays important role in public health, as cardiovascular disease and events related to external causes such as emergency situations are increasing in the general population. The reunion of historical information enables the reader to know how the creation and structuring of prehospital care services happened. The objectives were to describe the historical aspects of the structuring of the prehospital care service, its implementation and evolution in Goiás state. This update seeks to rescue historical data related to prehospital care services in Goiás. In Brazil the training of professionals and the way to assist the victims were based on the American and French models of prehospital care. They are public health policies related to emergency care that are new and need to go further and to be improved. As health professionals we are co-responsible for improving the quality of care.

    Fatores maternos e neonatais associados à prematuridade

    The prematurity is the most frequent cause of neonatal morbidity. The aim of this search was to perform a survey of the newborns with less than 37 completed weeks of gestation and related with the pathological alterations found. The data was collected from the maternal and neonatal medical record. The mean gestational age of the 104 newborns studied was 31 weeks ± 4 days. The mean weight of the premature infants in our study was 2350 grams. The groups with maternal baseline diseases were: maternal hypertension 51 cases (49%), uterus-placental disorders 21 cases (20.1%), infectious diseases 12 cases (11.5%), heart diseases 9 cases (5.7 %), diabetes 2 cases (1.9%), Acquired Immune Deficiency Syndrome 1 case (0.9%) and 8 cases (7.6%) with no record of the baseline disease. Among the groups with fetal baseline diseases the most frequent diseases were of the respiratory tract, with 81 cases (78%). In our study, there was statistically significant difference between gestational age and maternal baseline disease (p=0038). The prematurity remains the main cause of neonatal morbidity and mortality, being one of the biggest challenges for the provision of professional high quality assistance.

    Caracterização demográfica e morfométrica das síndromes hipertensivas da gestação

    The hypertensive disorders of pregnancy (SHG) have variable intensity according to their type. This study aimed to describe the demographic characteristics and maternal-fetal morphometric changes of placenta in different types of SHG by retrospective analysis of medical records of hypertensive patients with placentas evaluated morphologically, processed for immunohistochemistry using monoclonal antibody anti-human CD31 and analyzed morphometrically . The results showed that among the SHG, the groups with chronic hypertension (CH) and Pre-eclampsia superimposed on chronic hypertension (PSHC) showed higher maternal age (p = 0017). The frequency of the SHG was higher among multiparous. Lower gestational age, lower Apgar score and placental weights lower than expected for the population (p <0.05) were observed in cases with pre-eclampsia (PE) and PSHC. There was no difference in the number of vessels of placental stem villi. In the others, there was an increase in cases with PE and gestational hypertension (HGE) and reduction in cases with PSHC. The importance of the study is to describe the demographic and morphological SHG to improve the obstetric care and understanding of different types of SHG. The severity of symptoms of SHG is related to higher maternal and fetal complications and changes in placental vascular patern.

    Participação do acompanhante na humanização do parto e sua relação com a equipe de saúde

    The humanization is understood as a mutual responsibility between health services and the community, and it consists of reception and resolutivity. The objective of this paper was to think about aspects concerning the involvement of the partner in humanized childbirth care and their interface with the health staff. The reflexion was based in the researchers experience and in an up-to-date literature. The text was organized in two parts: the partner as a factor for the humanization of childbirth and the partner of childbirth and their relation with the staff. The humanized care is important to make the delivery a positive and safe experience. Among the measures of the humanized delivery is the women´s right of choosing the natural delivery, and the one of being accompanied. The relationship between partner and health staff is complex, considering the limitation of the former participation in the delivery. It is a process which is being constructed, and it involves physical-environmental aspects of the health institutions, professional qualification, and the own culture of the users which is, until nowadays, very submissive before their rights as citizens.

    Capacidade funcional em idosos na comunidade e propostas de intervenções pela equipe de saúde

    This study aimed to evaluate the activities of daily life (ADL) and instrumental activities daily life (IADL) of the aged registered in a Family Health Strategy in Goiania (GO) and to propose pertinent actions in case of total or partial dependency. It is a transverse and descriptive research, accomplished with 105 elderly in a team of the Strategy of Family Health. It was applied questionnaires and evaluation scales. By analysis of simple and percentile frequency of data. It was identified that the majority was female with 60 to 69 years old, married, catholic, house-owners, having an income of up to one minimum salary, illiterate and it has been related one to three pathologies. It was verified that 59,1% of old people with maximum independence for the ADL. 58,1% presented problems in performing IADL. Pertinent actions refer to stimulate physical activity, literacy and autonomy. This study showed that the most of elderly can take care of themselves at home but they can’t live independently in relation to IADL.

    Microbiota das mãos de mães e de profissionais de saúde de uma maternidade de Goiânia

    This descriptive study was carried out at a maternity of a University Hospital in Goiânia-Goiás, Brazil, between April and October of 2003. It aimed to identify the hand microbiota of mothers and health professionals delivering care to newborns. The study population consisted of all active health workers and the mothers receiving care at the service during the study period. From the hands of 31 subjects (15 mothers and 16 health professionals), Gram-positive organisms, Gram-negative rods and yeasts were isolated. The most frequently isolated microorganisms were: Staphylococcus aureus, Staphylococcus e negative-coagulase, Escherichia coli, Klebsiella pneumoniae, Citrobacter freundii, Enterobacter sp, Hafnia alvei, Serratia sp and Arizona sp, which literature has appointed as being associated with outbreaks of hospital infection in child-care and rooming-in units. This reveals the urgent need to elaborate permanent education programs for professionals and health service users about the importance of hand washing as a tool to prevent infection and guarantee quality care to newborns.

    Avaliação microbiológica de agulhas sistêmicas descartáveis de acupuntura

    The acupuncture is an invasive technique and the needles are the principal instrument. The objective of this study is to analyze the microbiological quality from different marks of disposable systemic acupuncture needles and to identify the contaminants microorganisms isolated. Study of 11 labels, corresponding to 30 different lots, a total of 1500 disposable systemic acupuncture needles were submitted to microbiological analyzed in Goiânia in 2007. The needles were inoculated in peptone water with aseptic techniques and incubated at 37ºC for 21 days. The tubes with growth were streaked in blood agar to isolate the microorganism. The colonies were identified by Gram stain and biochemical tests. The microbiological contamination was detected in 13 (0.86%) from the 1500 analyzed needles, corresponding to six labels, but the rate did not present any statistical significance. The microorganisms were identified as Gram positive bacilli, Micrococcus sp and negative coagulase staphylococci. The microbiological contamination rate did not present a statistical significance, and the analyzed needles should be considered security for using in acupuncture procedures.

    Identificação de diagnósticos de enfermagem do domínio segurança/proteção em idosos admitidos no sistema hospitalar

    In accordance with the necessity of rethinking the nurse performance towards to the health program of elderly people, it appears the necessity of knowing these people human answers at the moment of the hospital admission. This research had as objective: to analyze the nursing diagnosis profile (OF) of the domain Security/ Protection, of Nanda Taxonomy II, in the hospital admission of elderlies. The descriptive study was carried out in the West-Centre Region School Hospital medical clinic, from April to October, 2007. A previous validated protocol was used for data collection. For the data analysis it was adopted a clinical reasoning process proposed by Risner, and procedures of descriptive statistics. 28 aged people had taken part, 75% of these were masculine gender, 86,4% had between 60 to 79 years old, 71,4% married, 75% Catholics, 42,9% had studied from one to three years, 35,1% had an income up to two salaries. It was identified that 100% of these people had presented Harmed Dentition diagnosis, 100% Infection Risk, 57,1% Falls Risk, 14,2% Harmed Skin Integrity and 3,5% Hyperthermia. Inferring that these nursing diagnostics should be aimed as systematized nursing care, assuring aged people protection against present environment risks, whether in the hospital or in their houses, contributing with the hospital decrease internments and re-internments.
