Resumo

Dissertation produced in in the format of scientifics articles. The first article was submitted to the Ciência & Saúde Coletiva journal and is entitled “Burden’s caregivers and functional capacity of patients with strokes: sistematic review”. Objective: to investigate the changes in the functional capacity of stroke patients and their influence on the evolution of family caregivers' burden. Method: the following descriptors were used stroke and caregivers and burden; stroke and family and burden, in the Virtual Health Library (VHL) and in the United States National Library of Medicine (PubMED), considering articles published in the period from 2000 to 2016. Results: initially 420 titles were found; after the review stages nine longitudinal studies and a randomized clinical trial remained. The increased caregiver’s burden was associated with decreased in functional capacity of the patients. Other factors, including patient understanding and communication deficits, anxiety and depression in caregivers, elderly caregiver status, being female and patient's spouse, unemployment, lack of social support and family adjustment were also associated with increased caregiver’s burden. In studies where there was a reduction in burden, it was associated with improvement in functional capacity and training of the caregivers. The second article, to be submitted to the Latin-American Journal of Nursing, is entitled "Burden’s family caregivers and functional capacity of patients with strokes: longitudinal study”. Objective: to evaluate the burden of family caregivers and the their relation with the evolution of functional capacity of patients who suffered from Ischemic Stroke (IS) or Hemorrhagic Stroke (HS). Method: longitudinal analytical study, realized in three moments: in the first week of hospitalization, at discharge and three months after discharge. A questionnaire was used for the sociodemographic profile of caregivers, another for the sociodemographic and clinical profile of the patients; Functional Independence Measure (FIM) and Informal Caregiver Overload Assessment Questionnaire (COAQ). Results: participated 51 family caregivers of 51 patients with stroke sequelae; 29.41% with stroke and 70.59% with HS. Patients achieved significant gains in functionality in all domains, with some better outcomes in patients with stroke. At admission, moderate burden was observed in caregivers in most domains, with no changes in subsequent assessments. Significant changes were observed in the domain "Mechanisms of Efficacy and Control" in both types of stroke. The evolution of patients' functional capacity did not influence the evolution of the burden in the caregivers. Conclusions: the absence of the relationship between the patients' functional capacity and the burden in the caregivers demonstrates that changes in functional capacity can not be considered in isolation when analyzing the caregivers burden. The results direct the approach of rehabilitation professionals to the need for intensified multiprofessional care for patients with stroke sequelae that aim not only to improve functional capacity but also to adequately prepare caregivers.
