Avaliação citológica de material subamniótico para diagnóstico de corioamnionite: estudo de avaliação de teste diagnóstico

Resumo

Chorioamnionitis is a placental disease of infectious etiology, associated with a significant increase of maternal and fetal morbidity and mortality. Several studies have discussed the importance of its early diagnosis. The most widely used diagnostic approach is the analysis of amniotic fluid, obtained by amniocentesis (an invasive procedure, not inserted in the Brazilian prenatal routine) and the gold standard for its diagnosis is the histopathology of the placenta, but analytical and post-analytical phases of this exam may take enough time to delay the onset of therapy. This work aims for the validation of cytological evaluation of subamniotic material, through a diagnostic test study, comparing the cytologic evaluation with respectively histopathologic results. 125 samples (2 samples excluded) were evaluated cytologically and histologically, yielding a sensitivity of 77.55%, specificity of 60.53%, accuracy of 67.2% and predictive values of 55.88% (positive) and 80.7% (negative). Changing the cut-off for cytological positivity resulted in values similar to literature, however, the high specificity and high negative predictive value demonstrate low capacity to foresight histopathological chorioamnionitis, with the cytologic evaluation being a technique best used for the diagnosis of absence of chorioamnionitis.
