Resumo
Acute HCD is characterized by high
parasitemia, low leveis of circulating spespecific
antibodies and important acute inflamatory
process in the heart and other
organs. This phase hás a short duration
and is much more severe among young
children, chiefly under two years age.
Acute inflamation and severe dennervation
in this phase can result in important
consequences in the chronic stage, which
is characterized by low parasitemia, high
rates of circulating antibodies, chronic inflamation,
fibrosís and immune autoagresson
mechanisms.
The clinicai picture and the severity
of the disease can vary in different geographic
regions, possibly because of different
factors such as the parasite strain,
the inoculum, severity of the acute phase,
the host general conditions (inctuding
sex, age, nutrition, labour, intercurrences),
etc. The clinicai evolution is proteiform
but generally the chronic patients
remain several years or forever with an assymptomatic
(or olygosimptomatic) clinicai
pattern. Chronic Chagas' heart disease
can appear in 10 to 30% of the cases
with important consequences to the patients
lit"e and work capacity.
The medicai care system of the endemic
áreas need to have a very clear and
operative knowledge about the diagnostic
and therapeutic procedures on HCD. On
the other hand, the available prophytatic
measures must be emphasized in order
not only to reach the transmission control
but also to obtain possible benefits
on the morbidity and mortality pattern
of the disease in the region, at long or médium
term
