Resumo

Introduction: The Pharmaceutical Care and pharmaceutical intervention in primary health care in the control of hypertension in the elderly, allows identify resolving and preventing Drug Related Problems (DRP), and contribute to promoting the health and Rational Use of Drugs. The thesis was built in the form of scientific articles, and the first article is titled: " Drug Related Problems in elderly patients with hypertension ": an integrative review. The second is entitled: Impact of pharmaceutical care in monitoring of elderly hypertensive patients and evaluation of Drug Related Problems. Goals: First Article: review the literature to select, study and evaluate the publications that have focused Related Issues Antihypertensive medications in the elderly. According to article: Studying the impact of pharmaceutical care in monitoring of elderly hypertensive patients and evaluate the Drug Related Problems ". Method: Initially a search on the topic was conducted using the Pharmaceutical care descriptors, Drug Related Problems, hypertension and elderly in the databases MEDLINE, LILACS, Cochrane, IBECS, SciELO and PubMED, studies published between 1988 and 2013. The second article, a prospective, exploratory and quantitative study of 26 elderly hypertensive patients, followed for six months performed at a Basic Health Unit Family, in the north of the city of Goiania-GO. Patients were studied in two small areas: micro-area A, accompanied by the pharmacist monthly, and micro-area B only with the participation of community health worker, which only made the visits. In the first and last visit, the two micro areas were carried out assessment of the socio-demographic profile, performed the measurement of blood pressure of the study participants at the beginning and end of the study and identification of problems related to drugs in microarea A. Results: There are several studies describing on DRP and the importance of the pharmacist in the pharmacotherapeutic follow. Models of Pharmaceutical Care Dader Method and Model of Minnesota configure key facilitators models in DRP identification process. However it is concluded that there are few studies that address DRP in elderly hypertensive patients. Among the 26 elderly hypertensive patients, there was a predominance of female patients, aged 60 to 69 years, primary / secondary education, earning 2-4 minimum wages, married, white and retired. Reported not using alcohol, 74% did not exercise; 50% of the elderly were not any restriction of salt / sugar. Patients of micro-areas A and B are similar in terms of sociodemographic characteristics. The systolic blood pressure and diastolic blood pressure at baseline were similar in the two micro areas. At the end of the study patients of the micro area showed a significant reduction in mean systolic blood pressure and diastolic, and it was possible to detect DRPs in patients from the micro area, the most identified accession PRM in male patients, whose finding was significant . Conclusion: The pharmaceutical interventions are important to identify DRPs and may result in the reduction of complications of chronic diseases and reduction of morbidity and mortality. The Pharmaceutical Care was important to contribute to the control and reduction of blood pressure although it was not possible to establish the pressure reduction in relation to the identification of DRPs.
