Investigação soroepidemiológica da infecção pelo vírus da hepatite B em pacientes com diabetes mellitus tipo 2 em Goiânia-Goiás

Resumo
Hepatitis B and diabetes mellitus (DM) represent major public health issues.
Research has reported that diabetic patients are at increased risk for acquiring HBV,
development of chronic liver disease, cirrhosis and liver cancer. Nevertheless, there
is no data published related to this theme in Brazil. Thus, this is the first investigation
in our country with this population. The aim of this study was to investigate the
seroepidemiological profile of HBV infection in patients with type 2 diabetes
mellitus in Goiânia, Goiás. Diabetic patients from five public health units in Goiânia
city were invited to participate in the study. Thus, 605 patients were interviewed
about sociodemographic characteristics, vaccination against hepatitis B and factors
associated with HBV infection and the blood was collected, processed and stored
(serum) to -20°C in the Laboratory of Virology (IPTSP / UFG). All serum samples
were screened for the detection of HBsAg, anti-HBc and anti-HBs by enzyme-linked
immunosorbent assay (ELISA). Data were analyzed using the statistical program
STATA (13.1). The prevalence rates were calculated with 95% confidence interval
and was estimated the prevalence ratio (PR) of individuals exposed to HBV (antiHBc
positive) associated with the factors investigated. Logistic regression was
performed using the Poisson model. The anti-HBc alone was detected in 3.47% (95%
CI: 2.21- 5.34) of the subjects and, when combined with anti-HBs, the rate was
14.38% (95% CI: 11.73 to 17.48), resulting in an overall prevalence of HBV
infection of 17.85% (95% CI 14.92 to 21.19). Moreover, a low vaccination coverage
(10.41%; 95% CI: 8.15 to 13.19) and a high susceptibility rate to HBV infection was
observed (71.73%; 95% CI 67.93 to 75.25). A multivariate analysis of risk factors
showed that age >60 years, sharing of insulin vial, use of alcohol, paid sex and
multiple sexual partners were associated with HBV infection. These findings are
important to the context of health surveillance and may be used to support HBV
prevention, control and clinical management, which will reflect in better quality of
life and health care of the diabetic population.
