Fatores prognósticos relacionados à mortalidade em pacientes com esclerose lateral miaotrófica em ventilação não invasiva

Amyotrophic lateral sclerosis (ALS) is a neurodegenerative disease without curative treatment to date. Respiratory failure is the leading cause of mortality in the disease and noninvasive home ventilation is the most effective treatment. The objective of this study was to investigate prognostic factors related to death in patients with amyotrophic lateral sclerosis using noninvasive home ventilation. A retrospective cohort study was carried out in individuals with ALS in the home NIV, admitted to a specialized center for the treatment of neuromuscular diseases in Goiânia, Goiás. The medical records of 144 patients were evaluated through a standardized data collection form. The prevalence of death in the studied population was 56.9%, being associated with age ≥ 60 years (Odds Ratio adjusted 2.76, 95% CI 1.37-5.58, p <0.001), inability to perform spirometry (Adjusted odds ratio 2.34, 95% CI 1.17- 4.70, p = 0.02) and diurnal hypercapnia (unadjusted Odds Ratio 10.78, 95% CI 1.26-92.35, p = 0.03) . The time between the onset of symptoms and the institution of NIV proved to be a significant factor for the occurrence of death (p <0,007). These findings suggest that the earlier diagnosis of amyotrophic lateral sclerosis, reduces the mortality in this disease.
